What Lifespark COMPLETE Does

Lifespark COMPLETE model integrates the benefits of specialized geriatric in-home primary care services with ongoing, whole-person life coordination support that works hand-in-hand with our campus team to help our residents stay healthy, navigate their health care options with confidence, and ultimately live fuller, more independent lives as they age.

Our model provides two powerful additions to our on-site team: an in-home geriatric primary care expert and a dedicated Life Manager for ongoing coordination and guidance, providing additional guidance for the right services at the right time.

 How We Work Together

The Lifespark staff works closely with our campus staff as one COMPLETE team, collaborating and working cooperatively. Your Lifespark team members work to become integral connectors and contributors to your on-site team.

Each senior community has a dedicated in-home primary care provider (advanced practice professional: nurse practitioner/physician assistant) and Life Manager so you see the same people all the time.

They wrap around the work already being done in our community to help fill any gaps, extend our ability to do more for residents and families, and provide added support to address complex issues and concerns.
